On January 1, 2008, the
Advance Health Care Directive Act went into
effect.
The old Living Will and
Power of Attorney for Health Care forms have changed, and have been
combined into a single form called an Advance Health Care Directive.
The new form allows all Utahns aged 18 and older
to:
- Appoint an agent to
make health care decisions when they are unable to make or
communicate health care decisions
- Specify the powers of
the appointed agent
- Choose among options
for end-of-life care
